The SolutionHealth and its Member Entities (collectively known as “SolutionHealth”) have developed a patient portal (hereinafter “MyChart SolutionHealth”) service designed to (1) provide patients with Web-based access to portions of their medical records, (2) allow patients to communicate with staff through secure email and (3) offer parents, guardians, or other individuals concerned about the patient, proxy access to the patient's MyChart SolutionHealth account. For the following outlined Terms and Conditions, the words “you” and “your” shall refer to the individual accessing the MyChart SolutionHealth service. The words “us”, “we” or “our” shall refer to SolutionHealth.

By accessing or using MyChart SolutionHealth, you agree to be bound by the following “Terms and Conditions”. If you do not wish to be bound by these Terms and Conditions, you may not access or use MyChart SolutionHealth. We may modify these Terms and Conditions at any time, and such modification shall be effective once posted.

By clicking the “Accept” button you are agreeing to follow these Terms and Conditions and are indicating that you have read and fully understand these Terms and Conditions, therefore creating an electronic signature of acceptance.

We reserve the right to suspend or terminate access to MyChart SolutionHealth at any time and for any reason. If you have any questions about these Terms and Conditions, please contact the MyChart SolutionHealth Customer Support Team at MyChartSupport@solutionhealth.org.


1. SolutionHealth offers the MyChart SolutionHealth service as a convenience to patients. SolutionHealth assumes no responsibility for how you use the information you obtain from MyChart SolutionHealth, who you share your username and password with for MyChart SolutionHealth, or who you assign Proxy Access to under Section IV, Paragraph 3. MyChart SolutionHealth is not a substitute for appropriate and timely contact with your health care provider. You should never change or stop any course of treatment prescribed by your health care provider without first consulting him or her.

2. You should not use the MyChart SolutionHealth service if you have any emergency or think you need to speak to someone urgently. IF YOU BELIEVE YOU ARE EXPERIENCING A MEDICAL EMERGENCY, YOU SHOULD CALL 911 OR GO TO THE EMERGENCY DEPARTMENT OF YOUR LOCAL HOSPITAL.

3. Supplying an Email Address. You agree to register for MyChart SolutionHealth with a valid, current email address. If you change your email address, it is your responsibility to update MyChart SolutionHealth with the new email address.

4. SolutionHealth makes every effort to keep your health information secure and confidential. You also agree to do the following things to help keep your information secure:

You will keep your username and password secure When you register for MyChart SolutionHealth, you will create a username and a confidential password. SolutionHealth will treat anyone who uses your username and password as "you" and that individual will have access to all the information in your MyChart SolutionHealth record, including the ability to request your medical records through the patient portal and establish proxy access to your health information. Therefore, you are responsible for maintaining your username and password in strict confidence. In no event shall you share your username or password with any third party or allow another person to access MyChart SolutionHealth using your username or password. You shall notify your provider immediately if you suspect that someone is using your password in an inappropriate manner or if you would like a Proxy Access terminated.

You will never be asked for your MyChart SolutionHealth username and password in an email. Please do not respond to any emails that ask for this information.

If you have already responded to such an email, change your password right away and contact the MyChart SolutionHealth Customer Support Team at MyChartSupport@solutionhealth.org.

Email Notifications. You will be notified via email when there is new medical information to be viewed through your MyChart SolutionHealth account. Any person with access to your email will be able to see this notification. Although no private medical information is contained within the email notification, you may not want others to know. It is your responsibility to manage who has access to your personal email address.

5. SolutionHealth does not guarantee that you will be able to access MyChart SolutionHealth or, its availability at all times.

6. Confidentiality and Security. The privacy of MyChart SolutionHealth users’ health care information is protected by state and federal laws, including the Health Insurance Portability and Accountability Act (“HIPAA”). If you have questions or want to learn more about the privacy and security of your protected health information at SolutionHealth, please refer to the Notice of Privacy Practices.

7. Push Notification for the MyChart SolutionHealth Mobile App.If you download the mobile MyChart SolutionHealth application on your personal mobile device, when you have new information to view in MyChart SolutionHealth, such as a message from your doctor or a test result, you have the option to be notified in real-time with ‘push notifications’ on your mobile device. If you do not want to receive push notifications, you can configure the notification settings on your mobile device, similar to other mobile applications, to no longer receive push notifications.


1. Information submitted through MyChart SolutionHealth will become part of your SolutionHealth medical record. This includes messages sent by anyone that has proxy access to your MyChart SolutionHealth account. Email messages that you send through MyChart SolutionHealth will be routed to the appropriate department, and SolutionHealth or SolutionHealth affiliated staff, other than your healthcare provider, may review your message if appropriate based on their job functions.

2. You agree to read your MyChart SolutionHealth messages regularly. The messages you receive through MyChart SolutionHealth may contain important information.

3. You should not send messages requiring immediate attention through MyChart SolutionHealth. We will make reasonable efforts to be as responsive as possible to MyChart SolutionHealth messages. However, delays may occur based upon message volume, availability of staff, the complexity of the patient’s condition, or technology disruptions. Therefore, we cannot guarantee how long it will take us to respond to these messages and you should allow up to 2 business days for a response. SolutionHealth is only able to respond to electronic communications based on the information provided by you. If there is insufficient information provided, we will be unable to provide a reliable or accurate response.

4. Prohibited Uses of MyChart SolutionHealth. You agree that you will not upload or transmit any communications or content of any type (including secure messaging) that may infringe upon, misappropriate or violate any rights of any party and that your use of MyChart SolutionHealth will not include any of the following:

•Signing on as or pretending to be another person;

•Transmitting messages or material that infringes on or violates the intellectual property rights of others or the privacy or publicity rights of others;

•Transmitting material or content in any form or media, including email or other electronic communication that is unlawful, obscene, defamatory, predatory of minors, threatening, harassing, abusive, slanderous, or hateful to any person (including SolutionHealth personnel) or entity;

•Using MyChart SolutionHealth’s interactive services in any way that is intended or is reasonably likely to harm the patient or others; or

•Intentionally distributing viruses or other harmful computer code.

Medical Record Data

1. The MyChart SolutionHealth service provides online access to portions of your SolutionHealth electronic record. Not all of your SolutionHealth medical record may appear on your MyChart SolutionHealth record. Posting of information is at the discretion of your individual provider(s) and SolutionHealth.

2. Your health care provider(s) will maintain and update your electronic medical record at each office visit. However, if you believe any information displayed in MyChart SolutionHealth is not accurate, you are encouraged to notify your health care provider at your next appointment or by calling your provider’s office.

3. You may request copies of your medical records through the MyChart SolutionHealth and have them sent to you at through the patient portal.


You may request proxy access through SolutionHealth for your minor child(ren), and an adult(s) for which you are the court appointed legal guardian of as outlined below in Paragraph 1 and 2. By accepting these Terms and Conditions in their entirety, you certify that you are authorized to open and/or access the account you are requesting access to and that all information you have provided to SolutionHealth is true and correct to the best of your knowledge. For all types of proxy access/accounts, SolutionHealth reserves the right to modify, limit or revoke proxy access at any time for any reason, with or without notice to the account holder, proxy or anyone else.

1. ChildrenParents or legal guardians of children ages 0-11 can have full access to their child’s record in MyChart SolutionHealth by filling out and submitting the MyChart SolutionHealth Parent/Guardian Access Form. Once the child turns 12, your access will automatically terminate and you will need to sign a new form for new limited access. Parents or guardians of children 12-17 years of age can have limited access to the child’s MyChart SolutionHealth account. Once the child turns 18, your access in its entirety will automatically terminate. Please refer to the MyChart SolutionHealth FAQ for additional information. The minor patient may also request a personal MyChart SolutionHealth account once the child turns 12 years of age.

2. Adult Patients (18+) – Court Appointed Legal GuardianThe only proxy access that will be created directly by a workforce member of SolutionHealth will be for adult(s) who are the legal guardian of the patient(s). If you are 18 years or older and would like to grant another competent adult proxy access to your MyChart SolutionHealth account, you must follow the steps outlined below in Paragraph 3. SolutionHealth will not be responsible for directly establishing these types of accounts and the responsibility will be on you to grant another individual(s) proxy access to your patient portal account.

Court appointed legal guardian(s) may request proxy access to another individual’s MyChart SolutionHealth account. To obtain proxy access the legal guardian must fill out and submit the MyChart SolutionHealth Proxy form. You further agree that in the event you request proxy access to an individual(s) account you will be required to provide documentation of your authority (for example, court documentation you were appointed guardian) before you can receive an access code to complete registration or access that individual(s)’s personal health information. Access to another adult’s account is good for 365 days, at which time your access will terminate, and you will need to complete the MyChart SolutionHealth Proxy form and certify you are still the individual(s) appointed legal guardian. SolutionHealth reserves the right to revoke proxy access at any time.

3. Adult Patients (18+) - Other Proxy AccessIndividual(s) 18 years or older may grant another individual(s) proxy access to their patient portal directly through the patient portal. If you would like to grant another individual(s) proxy access to your patient portal, you may do this by going to the “Health” tab, clicking “Share My Record”, then choosing “Friends and Family Access”, click “Invite Someone”, and fill out the appropriate information to send the individual(s) of your choice an invitation for access to your health record. The individual(s) you invite MUST be a patient of SolutionHealth. The proxy access for this individual(s) will last for 365 days. If you want to terminate this individual(s) proxy access you MUST notify your provider. You understand and agree that you are solely responsible for the accuracy of information you enter into the patient portal and the selection of the appropriate proxy individual(s) when sending a proxy access invitation. You further agree that if you grant proxy access to other individual(s) through this process you release SolutionHealth from any legal responsibility of liability relating to the access and release of your protected health information to the individual(s) you granted proxy access to.


Subject to the Terms and Conditions contained herein, SolutionHealth grants you a limited license to access and use MyChart SolutionHealth. This license is for your personal use only and may not be shared with or transferred or sublicensed to any other party.
YOU AGREE THAT NEITHER WE, NOR OUR OFFICERS, DIRECTORS, EMPLOYEES, AGENTS, CONTENT PROVIDERS OR SUPPLIERS SHALL HAVE ANY LIABILITY TO YOU UNDER ANY THEORY OF LIABILITY OR INDEMNITY IN CONNECTION WITH YOUR USE OF MYCHART SOLUTIONHEALTH. YOU HEREBY RELEASE AND FOREVER WAIVER ANY AND ALL CLAIMS YOU MAY HAVE AGAINST US, OUR OFFICERS, DIRECTORS, EMPLOYEES, AGENTS, CONTENT PROVIDERS OR SUPPLIERS (INCLUDING BUT NOT LIMITED TO CLAIMS BASED UPON OUR NEGLIGENCE, OR THE NEGLIGENCE OF OUR OFFICERS, DIRECTORS, EMPLOYEES, AGENTS, CONTENT PROVIDERS, OR SUPPLIERS) FOR LOSSES OR DAMAGES YOU SUSTAIN IN CONNECTION WITH YOUR USE OF MYCHART SOLUTIONHEALTH.

Why won't MyChart let me log in? ›
