Birth and Death records are on file and can be provided by the Vital Records Department upon written request accompanied by proper identification and payment by cash, check, or money order. The cost of death certificates is $15.

In order to receive a general copy or certified copy of a birth certificate from the Clark County Health Department, you must present acceptable forms of Identification and a completed Certified Birth Certificate Form.
